The RISE method
A hazard report, an incident, an audit finding, an inspection result, a near miss, a concern somebody raised in a meeting. Whatever gets raised is given an owner and a date and pushed until it is verified and closed.
01 Capture
A phone in a hangar, a QR code on a wall, a tablet on the job site. Reporting is simple. If raising something is hard, it does not get raised, and a system with nothing in it tells you nothing.

02 Evaluate
