Ten questions, five minutes each, answered anonymously. Compare the average against your own score and the distance between the two is the finding.

How to run it

Hand it out, add it up, read the distance

1Running itHand it to everyone doing the work, not just supervisors. Tell people plainly what it is for and that nothing on it is attributable. Collect them in a way that keeps it that way: a box on a wall beats an email thread.
2Scoring itYes is 2, Partly is 1, No is 0, so each sheet totals out of 20. Add every sheet and divide by the number of sheets. A blank counts as nothing on both sides of the division.
3Reading itThe average is the number to set beside your own. A leadership score of 32 against a crew average of 11 says something neither number says alone, and the questions furthest apart are where to start.
